Patient Portal Accounts

Each patient in the practice is given a unique Patient Portal for secure electronic transmission of patient history, information, and questionaires.  The information provided is securely and seamlessly transmitted to the doctor for review during the upcoming appointment and interfaces directly with the patient's electronic medical record.  Assigned documents are available to the patient and their families between appointments to allow for completion on the family's schedule.  Information gathered in this way increases efficiency, improves information transfer from patient to doctor, and increases precision of the medical documentation with the goal of improving the patient's outcome.  Through their portal, patients and families are also able to update demographic information, view upcoming appointments, and review practice documents.

New Patients

Please review the new patient information, then contact the office to get registered and to schedule an initial appointment for evaluation.  Your individual patient portal account will be established at that time and you will be given a validation code to use for initial log in. 

Within 24 hours of initial registration with Dr. Herdman, you will need to complete the Patient Portal account set-up.  

Follow these steps to establish a patient portal account:

1.) the patient's last name

2.) your validation code

Validation codes are time sensitive and must be used within 24 hours of initial registration.  Please adhere to this timeframe to prevent account lock-out.

3.) the words/ numbers you see in the red box

  • Please enter the words you see in the box, in order and separated by a space. Doing so helps prevent automated programs from abusing this service.

  • If you are not sure what the words are, either enter your best guess or click the reload button next to the distorted words.

  • Visually impaired users can click the audio button to hear a set of words that can be entered instead of the visual challenge.

Please Note: If portal is not established and/ or all requested portal documents are not completed PRIOR to the start of the initial evaluation appointment, it will be necessary to reschedule the evaluation for a later date to allow time for completion of this necessary documentation.  Cancellation/ no show policy will be in effect in such circumstances.

Access to your patient portal will be required periodically throughout treatment.  Please keep your portal log in and password in a secure and accessible location for future use.
